Please use the checklist below as a prompt to help you with each of the stages in supporting a Carer or Young Carer. It also gives links with full details of the resources available to provide support.

Have you identified and recognised a Carer who is looking after your patient?
Are there any children in the household who are providing care?
What support have you offered the Carer? For example
Have you referred the carer to:
Social Care - Carers Assessment
And / or
Local Carers Services via the Surrey Carers Prescription

Have you referred the Carer for any Enhanced Services?
Referral to Surrey County Council: Adult Social Care or Children's Services, for Carers Needs Assessment via SCC Contact Centre - 0300 200 1005 (fax 020 8541 9004) may be subject to eligibility criteria)

Crossroads Care Surrey - provides a flexible domiciary replacement care services, and an End of Life Carers Support Service for people with a life limiting condition
Borough and District Councils, for housing support including Adaptations and Telecare
Age UK Surrey - provide a range of services and activities with older people in mind including domestic help, gardening, befriending shopmobility and footcare
Have you checked the carer is willing and able to care?
Have you provided the carer (oryoung carer and their family) with relevant information.
Emergerncy Carers card - This card is available to all Carers in Surrey. To obtain stocks of thsi card call 01483 519146
